By assisting in identifying opportunities to promote partnerships with professional and referral sources, the coordinator helps strengthen community ties and expand the reach of services. Coordinating with prospects, family members, caregivers, and other professional providers ensures a seamless transition into the community, while also referring residents and patients to Lutheran Life Communities' services when appropriate, fostering ongoing support and care.
Schedule Varies: This is a full-time position Monday-Friday, some weekends and holidays are required
