What You’ll Do:

  • Coordinate the full admission and transfer process for residents, ensuring smooth transitions and positive experiences
  • Lead the Social Services department, supervising and developing team members
  • Complete and maintain social service assessments, care plans, and MDS documentation
  • Provide support and advocacy to residents and families to enhance well-being and quality of life
  • Serve as the abuse and neglect coordinator, ensuring compliance with state and federal regulations
  • Collaborate with interdisciplinary teams to drive quality and service excellence

Schedule:

Full-time, salaried position
Day hours with weekend rotation required

What You’ll Need:

  • Licensed Clinical Social Worker (LCSW) certification required
  • Minimum 3 years of experience in a Skilled Nursing Facility (SNF) with 100+ beds
  • Strong leadership, communication, and organizational skills
