How you will make an impact:
A Care Coordinator makes a significant impact by promoting the Ministry, Mission, and Core Values of The Lutheran Home and Lutheran Life Communities through professional, ethical practices and exceptional hospitality. By assessing potential clients’ needs, developing effective service plans, and overseeing the delivery of MySolutions home care and handyman services, the Care Coordinator ensures personalized, high-quality support. Through home visits, ongoing assessments, 90-day check-ins, and annual care plan reviews, they foster strong relationships with clients, families, and caregivers. Their holistic approach integrates direct and indirect services, community resources, and advocacy to exceed client satisfaction and enhance overall well-being.
Schedule Varies: On call as needed; normally 8 to 4 pm M-F but occasional evenings and weekends
What you will need:
- College degree preferred;
- Sales, Healthcare or Social Work background a plus.
- Supervisory experience a plus.
- Must be able to read, comprehend and write legibly in English.
- Must be able to comprehend and perform oral and written instructions and procedures.
- Must be able to calculate math figures and amounts.
