The Lutheran Home, part of Lutheran Life Communities, is hiring a Director of Social Services to lead the Social Services department within our skilled nursing community. This leadership role focuses on supporting residents and families, coordinating care transitions, and ensuring high-quality psychosocial support for those we serve.

What You’ll Do

  • Coordinate the full admission and transfer process for residents, ensuring smooth transitions and positive experiences
  • Lead and supervise the Social Services department while supporting and developing team members
  • Complete and maintain social service assessments, care plans, and MDS documentation
  • Provide support, guidance, and advocacy for residents and families to enhance well-being and quality of life
  • Serve as the abuse and neglect coordinator while ensuring compliance with state and federal regulations
  • Collaborate with interdisciplinary teams to support quality care and service excellence
  • Help ensure residents’ psychosocial needs are addressed and supported appropriately

Required Qualifications

  • Minimum of three years of experience working in a Skilled Nursing Facility with 100+ beds
  • Strong leadership, communication, and organizational skills
  • Experience supporting residents and families in a healthcare or long-term care environment
  • Ability to collaborate effectively with interdisciplinary care teams
