This leadership role focuses on supporting residents and families, coordinating care transitions, and ensuring high-quality psychosocial support for those we serve.

What You’ll Do
- Coordinate the full admission and transfer process for residents, ensuring smooth transitions and positive experiences
- Lead and supervise the Social Services department while supporting and developing team members
- Complete and maintain social service assessments, care plans, and MDS documentation
- Provide support, guidance, and advocacy for residents and families to enhance well-being and quality of life
- Serve as the abuse and neglect coordinator while ensuring compliance with state and federal regulations
- Collaborate with interdisciplinary teams to support quality care and service excellence
- Help ensure residents’ psychosocial needs are addressed and supported appropriately
Required Qualifications
- Minimum of three years of experience working in a Skilled Nursing Facility with 100+ beds
- Strong leadership, communication, and organizational skills
- Experience supporting residents and families in a healthcare or long-term care environment
- Ability to collaborate effectively with interdisciplinary care teams
