How you will make an impact:
Join our team as a Social Services Coordinator II and make an impact by coordinating outside psychiatric and psychosocial services to assist with behavior modification plans, ensuring comprehensive care for residents. You will assess resident needs through social service interviews, provide counseling, advocacy, discharge planning, and access to community resources. As a social services coordinator, you will encourage resident participation in individual and group programs, both inside and outside of the facility. You will facilitate family involvement in care plans and assist residents with legal documents, ensuring seamless transitions and ongoing family support. Additionally, you will meticulously document social service plans and progress notes in resident charts, reflecting goals and interventions effectively.
Schedule : This position is full time Monday-Friday, 8am-4pm with a weekend rotation.
What you will need:
- Experience in geriatrics preferred
- Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds preferred
- Related experience and/or training; or equivalent combination of education and experience.
- 3 years of social services experience in a Long-Term Care setting preferred
